Instructions:

This tool helps assess behavioral and emotional concerns in children aged 6–18 years over the past month. Answer the questions honestly based on how often you observe these behaviors in your child.

0 = Never

1 = Sometimes

2 = Often

Each response earns points. Add up your score at the end to interpret the results.

Interpretation of Results:

0–7 Points: Minimal Concerns

Your child’s behavior and emotions appear typical for their age. Monitor their progress and provide support as needed.

8–14 Points: Mild Concerns

Some behaviors may need attention. Open communication, parental involvement, and small lifestyle changes (e.g., routine, playtime) can help.

15–22 Points: Moderate Concerns

These behaviors may be affecting your child’s social, emotional, or academic life. Consider seeking professional guidance for early intervention.

23+ Points: Significant Concerns

Your child may need immediate support to address emotional or behavioral challenges. Professional assessment and counselling are strongly recommended.
