Purpose:
The Everyday Memory Questionnaire (EMQ) evaluates how frequently you experience memory lapses in everyday life. This simplified version is tailored for easy scoring and self-assessment.
Instructions:
Rate how often you experience the following situations in your daily life:
0 = Never
1 = Rarely
2 = Sometimes
3 = Often
4 = Always
At the end, add up your total score to interpret your results.

Interpretation of Results:
0–12 Points: Low Everyday Memory Issues

13–24 Points: Mild Everyday Memory Issues
Occasional lapses may occur. Consider strategies like using reminders, note-taking, or reducing multitasking.
25–36 Points: Moderate Everyday Memory Issues
Memory lapses may be interfering with daily life. Focus on improving concentration and stress management techniques.
37–48 Points: High Everyday Memory Issues
Frequent memory lapses may indicate underlying issues like stress, fatigue, or health conditions. Consult a healthcare provider for further evaluation.
