Purpose:
The Cognitive Failures Questionnaire (CFQ) assesses how often you experience lapses in memory, attention, and action in daily life. This simplified version is tailored for easy scoring and self-assessment.
Instructions:
Rate how often you experience the following situations in your daily life:
0 = Never
1 = Rarely
2 = Sometimes
3 = Often
4 = Always
At the end, add up your total score to interpret your results.

Interpretation of Results:
0–10 Points: Low Cognitive Failures
You experience minimal cognitive lapses. Maintain a healthy routine and continue practicing focus and mindfulness.
11–20 Points: Mild Cognitive Failures
Occasional lapses may occur but are manageable. Consider using reminders, improving sleep, or reducing multitasking.
21–30 Points: Moderate Cognitive Failures
Cognitive lapses may be impacting your daily life. Focus on improving concentration techniques and managing stress.
31–40 Points: High Cognitive Failures
Frequent lapses could be a sign of significant distraction, stress, or health issues. Consulting a healthcare provider may help identify underlying causes.
